Admin request leak on failed Identify NS exhausts pool and panics kernel
Summary
In nvme_admin_state_identify_ns when per-namespace Identify command completes with non-zero error status error path executes continue without calling nvme_put_request(req). Each failure permanently removes one request from admin submission queue first_avail free-list. Admin pool only maxqe-1 slots (<=255). Device advertising many namespaces and failing each Identify NS exhausts pool next nvme_get_admin_request trips KKASSERT(queue->qid!=0) in nvme_get_request admin qid is 0 fatal kernel panic during controller attach. Device-controlled ns_count capped to 1024. Contrast sibling exit paths DO release: nscary-full at :653 success at :671. Also reachable by benign-but-buggy firmware advertising namespaces it cannot describe.

Verdict
NOT REPRODUCED (hardware / trigger gated). The cited vulnerable code path exists
in sys/dev/disk/nvme/nvme_admin.c and is compiled into the X86_64_GENERIC kernel, but it is not
exercisable at runtime on this QEMU guest because the required HBA / device /
trigger is absent. Confidence in the source bug itself: certain.
Why it cannot be reproduced here (the gate)
Hard-gated: needs an NVMe controller. The guest has NO NVMe controller; nvme_admin_state_identify_ns never runs.

- A defense-in-depth fix.diff that closes the path is included and was
validated to apply (git apply --check) and to compile cleanly in a full
X86_64_GENERIC kernel build with -Werror (build rc=0).
Fix
Call nvme_put_request(req) on the failed-Identify-NS error path before continue, matching the success and nscary-full exit paths which do release the request.
The standalone git-apply-able diff is fix.diff.
Reproduce
./build.sh && ./run.sh β both are no-ops on this guest by design (the gate
holds). Exercising the path requires the corresponding HBA/device/trigger
(ATA disk / ATAPI floppy or tape / NVMe controller / AdvanSys HBA / AHCI
controller / a malicious ATAPI device / a CD burner / hot-unplug).

NOT REPRODUCED (HW/trigger-gated on this guest). The bug is REAL in source (traced line-by-line): nvme_admin admin request leak on failed Identify NS (no NVMe controller). Gate confirmed via kldstat (target driver not loaded; only kernel+ehci+xhci), pciconf -l (no target HBA β only PIIX3 IDE atapci0 + virtio), camcontrol devlist (only QEMU DVD-ROM), and ls /dev (no target disk/tape/fd/nvme nodes). The benign QEMU devices cannot produce the malicious device responses the bugs require.
